What shapes the scope
Oversight needs vary with the practice and the work involved. Discuss these factors so the proposed engagement reflects actual responsibilities:
- Services, devices and the clinical complexity of the treatment menu.
- Provider roles, experience, availability and training needs.
- Locations, on-site participation and consultation coverage.
- Chart review, protocols, documentation and onboarding work.
- Whether the practice is opening, expanding or changing its current arrangement.
Assessing local fair market value
We assess local fair market value for the medical director’s practice-of-medicine services based on the local market, the agreed service scope, the time involved and the physician’s clinical responsibilities. The assessment and proposed compensation should be documented around the work to be performed, rather than referral volume.
The scope may include:
- Chart reviews and on-site clinical training.
- Protocol review and adverse-event management.
- Clinical orders.
- Clinical review and approval of online wellness and prescription medication services, with product-specific prescribing, sourcing and clinical review; listing a service here does not confirm that a particular drug or compounded product is available or lawful.
- Performing good-faith exams and coordinating their documentation with ongoing care.
Illinois pricing is determined individually. Request a written scope and proposal based on the work, time and responsibilities involved. Pricing and onboarding timing are confirmed for each engagement.
$10 good-faith exams within the practice relationship
The $10 per-exam offering is for participating practices within an established clinical relationship. Exams are connected to clinical oversight and documentation in our proprietary electronic medical record system. We do not offer standalone exams to outside organizations.
The exam price is not a quote for the full oversight engagement. Discuss the overall scope, applicable services and written pricing through the same professional inquiry process.
Match oversight to the services
Budget for the work the treatment plan actually requires: qualified assessment, appropriate prescribing authority, consultation, documentation, emergency response and procedure-specific on-site coverage. The laser rules distinguish ablative and non-ablative care. APRN full practice authority and PA collaboration also require different arrangements.
Do not treat a fixed exam percentage, visit schedule or director fee as an Illinois compliance standard. Any internal participation policy belongs in the engagement terms and does not replace the applicable law.

From inquiry to engagement
Discuss any proposed Spa Shield participation, operating tools and internal requirements while reviewing fit. These are engagement terms, not Illinois statutory approvals.
- Introduce your needs. Select your professional persona and tell us about your practice, client or clinical interests.
- Review fit. Discuss structure, services, qualifications, availability and the support required.
- Review the proposal. Clarify responsibilities, pricing, payment terms, exclusions and any transition or termination provisions before signing.
- Prepare to begin. Complete the agreed credential review, clinical arrangements, documentation and operational onboarding before affected work begins.
Questions to settle before signing
- Which activities and locations are included?
- Who handles clinical questions and after-hours escalation?
- What requires an additional discussion or amended scope?
- How are changes, notice and records responsibilities addressed?
Submitting an inquiry does not create an engagement, reserve coverage or authorize a charge. Fees other than the stated participating-practice exam price are discussed individually. There is no guaranteed onboarding timeline, clinical outcome or provider income.
Review compensation independently
Written terms should reflect actual services and responsibility. Illinois restricts fee splitting, including certain percentage payments for marketing or management. Discuss the proposed structure with counsel; fair market value alone does not resolve every restriction.
